COBS ensures that at all times the individual remains the centre of all decision making/planning regarding their programs at COBS.
COBS staff work closely with the individual, their advocate and any other person the individual wants involved, to ensure all goals and outcomes are met.
COBS management staff will hold a meeting with the above people annually, however a meeting can be held at any time upon request from the individual.
To ensure that each individual has their goals set out clearly and in a format that is easy to read, COBS utilise the following proformas.
Once a year (or on request) an Individual Service Plan (ISP) meeting is held. This is to set and/or review goals around the individuals life goals and about what they want to achieve.
The COBS Manager - Client Services will initiate a meeting with the individual, their primary carers, family members, professionals, and other relevant parties to discuss these goals. The individual decides who attends these meetings.
For the programs individuals participate in at COBS, an Individual Program Plan (IPP) is prepared and goals are set.
The goals are usually specific to a program (eg. cooking, Rural) however some goals may cross several programs (eg. Travel Training, Tieing shoe laces or making a cuppa).
These goals are reviewed at the end of six months or sooner if requested.
The individual and program staff are involved in this planning activity.
